This email with the subject "Important - Please complete attached form" was received in one of Scamdex's honeypot email accounts on Thu, 16 Jan 2014 03:55:04 -0800 and has been classified as a Generic Scam.

The sender was "Bankline" <>, although it may have been spoofed.

This message has been scanned by the Bankline CSC SSM AV and found to be free
